Polio Cases in Pakistan 2017 — The Lowest Total on Record at the Time

Pakistan recorded eight wild poliovirus cases in 2017 — at the time, the lowest annual total in the country's history, and a reduction of more than 97 per cent from the 306 cases of just three years earlier.

A genuinely small number

Eight cases in a country of over 200 million people meant that wild poliovirus transmission had been pushed into a handful of communities. Cases were reported from the established reservoirs — parts of Khyber Pakhtunkhwa and the tribal belt, Balochistan's Quetta block, and Sindh — rather than appearing in new territory.

Why the programme stayed cautious

The case count told only part of the story. Environmental surveillance through 2017 continued to find poliovirus in sewage samples from multiple cities, including places that had reported no paralysis cases at all. Because roughly one infection in two hundred causes paralysis, widespread environmental positivity alongside a very low case count is exactly what silent transmission looks like.

The programme's technical advisory group was explicit at the time that low case numbers reflected high population immunity suppressing paralysis, not the elimination of the virus. That judgement was vindicated within two years.

The immunity question

Repeated campaigns build immunity in the children who are reached. They do nothing for the children who are consistently missed — and in 2017 those children were still concentrated in the same union councils, in the same mobile and underserved populations. A low case year with an unchanged missed-children population is a fragile achievement, because the susceptible pool keeps refilling as new babies are born.

What came next

2018 brought 12 cases, and 2019 brought 147 — a more than tenfold increase driven by falling campaign quality, rising refusals and a wave of vaccine misinformation. The 2017 low point is therefore studied within the programme less as a success than as a warning about how quickly gains reverse.
